One example is Salaama Adult Day Center LLC, also based in Minneapolis. State records indicate its license was renewed on January 1, 2026, with the next renewal not due until December 31, 2026. Like Samara, it advertises services for adults requiring supervision and daytime assistance. Yet Rose’s on-the-ground observations, supported by community tips, suggest little to no client presence.
That disconnect between regulatory status and observable reality has fueled suspicions that some facilities may exist primarily to bill Medicaid or related programs, rather than to provide care.
State officials have not publicly accused Samara or Salaama of criminal wrongdoing. Still, the optics have proven difficult to ignore, particularly as Minnesota grapples with a documented surge in questionable providers.
A Statewide Crisis Years In The Making
Rose’s reporting landed at a sensitive moment for Minnesota regulators. In December 2025, the Minnesota Department of Human Services announced a sweeping two-year moratorium on new adult day care licenses, set to begin February 1, 2026, and run through January 31, 2028.
According to DHS data cited by CBS News, the number of adults receiving day services in Minnesota has increased by just 7 percent over the past decade. During that same period, provider capacity ballooned by 43 percent.
Temporary DHS Commissioner Shireen Gandhi said the mismatch raised red flags about overcapacity and potential abuse. The pause, she explained in a public statement, is intended to give regulators time to focus on program integrity and ensure existing providers comply.
Federal prosecutors have already brought criminal charges against operators in adjacent Medicaid-funded sectors, including autism services and housing stabilization programs. Adult day care, long viewed as lower risk, has increasingly drawn attention as patterns of alleged billing fraud emerge.
Still, officials have emphasized that not all providers are suspect. Many centers serve vulnerable populations with professionalism and care. The challenge, as one DHS source told local media, is identifying which operations are legitimate and which may be exploiting systemic blind spots.

What sets the Samara case apart is the role of social media in accelerating oversight pressure. Rose’s footage did not rely on leaked documents or anonymous tips alone. It showed empty rooms, quiet hallways, and unanswered questions, presented plainly and repeatedly.
That visibility changed the equation. Within hours of Musk’s repost, Rose’s investigation was being discussed by commentators, policy analysts, and everyday users, questioning how such facilities remain licensed.
Still, experts caution against trial-by-viral-post. Regulatory enforcement requires audits, subpoenas, and due process, not just compelling video. Yet, as one former state investigator told regional press, public attention often forces agencies to prioritize cases that might otherwise linger.
For now, DHS has not announced a specific probe into Samara Adult Day Care. But officials acknowledge that citizen reporting, including digital documentation, increasingly plays a role in identifying compliance concerns.

As Minnesota enters its licensing moratorium, existing adult day care centers will face heightened scrutiny. Inspections, billing reviews, and enrollment verifications are expected to intensify, according to DHS guidance issued in December.
